Chapter 460: Death In The Arctic, Under Pressure, And Sideswiped!
"Hello there viewers...Keiko here again...*Keiko proceeds to go and currently say this while traditionally bowing to the viewers as per usual. To which Keiko then proceeds to go and currently bring her attention and as well as her eyesight back up again, in order to go and continue to address the viewers as per usual. Keiko also now proceeds to go and briefly clear her throat, as Keiko then proceeds to go and continue, to properly address the viewers as per usual*...
Now then viewers, for this chapter I will once again be covering several more transportation related accidents in history...And for this particular chapter viewers as like the last one, we will be covering another three of them...
And each of them, as shown by the chapter title of this particular chapter, is going to be covering three aviation accidents, one of which was attributed in some way, to a case of 'pilot error leading to controlled flight into terrain'...
Another of which was attributed in some way, to a case of 'an in flight fire'...
And another of which was attributed in some way to a case of 'an instrument malfunction leading to pilot error and an in flight breakup'...
Now then viewers, lets begin with the first of the three aviation accidents...
And for this one, we will start by going all the way back, to August 20, 2011...
And on this particular day...First Air Flight 6560...Which was a domestic charter flight that was operating a service from Yellowknife, in the Northwest Territories of Canada, to Resolute Bay Airport, in Resolute, Nunavut, Canada. And on this particular day, the aircraft that was being used to operate the flight, was a Boeing 737-210C, and like that of the Boeing 747 that was operating as South African Airways Flight 295, was a combi, or a combination aircraft...
And on board First Air Flight 6560, were two pilots, 2 flight attendants, and 11 passengers...
The captain of First Air Flight 6560, was Blair Rutherford, who was the pilot flying, and had been flying for First Air for about 15 years...
And the first officer of First Air Flight 6560, was David Hare, who was the pilot monitoring, and had been flying for First Air for about 4 years...
And as for what wound up happening with regard to the crash that would result involving First Air Flight 6560?...
Well on approach to Resolute Bay Airport, Captain Rutherford wound up inadvertently moving his control column during the turn onto the final approach track. Which tragically wound up causing the autopilot to disengage from VOR/LOC mode. And then revert to only maintaining the current heading, which wound up resulting in the aircraft drifting out to the right of the runway centreline...
And tragically this wound up with First Air Flight 6560 crashing into a hill less then a mile from the runway, with the crash killing all 3 crew members, and all but 3 of the 11 passengers on board...
And now viewers, we will be moving onto the second of the three aviation accidents of this chapter...And for this one, we will be going all the way back, to July 11, 1991...
And on this particular date Nigeria Airways Flight 2120, which was a chartered passenger flight operating from Jeddah, in Saudi Arabia, to Sokoto, in Nigeria...
And on this particular day, the aircraft operating the flight was a Douglas DC-8-61 operated by Nationair Canada for Nigeria Airways...
And on board Nigeria Airways Flight 2120, were 3 pilots, 7 flight attendants, a purser, an in-flight service director, a lead mechanic, a project manager, and 247 passengers. For a total of 247 passengers and 14 crew members...
The Captain of Nigeria Airways Flight 2120, was 47 year-old William Allan, who was a former Royal Canadian Air Force pilot, and had logged 10,700 flight hours with 1,000 of those hours being on the DC-8...
The First Officer of Nigeria Airways Flight 2120, was 36 year old Kent Davidge, who had logged 8,000 flight hours, of which 550 of those hours were on the DC-8...
And the Flight Engineer of Nigeria Airways Flight 2120, was 46 year old Victor Fehr, who had logged 7,500 flight hours, of which 1,000 of those hours were on the DC-8...
And as for the cause that wound up leading to the crash of Nigeria Airways Flight 2120?...
Well on the takeoff roll from Jeddah, at least two of Nigeria Airways Flight 2120s main landing gear tires wound up blowing out due to them being underinflated...
And with no censors in the wheel wells, just three seconds after Nigeria Airways Flight 2120 took off from Jeddah, the landing gear wheels that were still on fire, were retracted into the wheel well...
And after about ten minutes later and as well as a raging in flight fire, Nigeria Airways Flight 2120 wound up crashing into the ground just short of Jeddah Airport. With the impact killing all 247 passengers and 14 crew members instantly...
And to this day Nigeria Airways Flight 2120 is the deadliest accident involving a DC-8 and the deadliest aviation disaster involving a Canadian airline...
And now viewers, we will be moving onto the third and final aviation accident of this chapter...And for this one, we will be a bit forward in time, to June 6, 1992...
And on this particular date, Copa Airlines Flight 201. Which was a regularly scheduled passenger flight from Tocumen International Airport in Panama City, Panama, to Alfonso Bonilla Aragón International Airport in Cali, Colombia...
And on this particular day, the aircraft operating the route was a Boeing 737-204 Advanced...
And on board Copa Airlines Flight 201, were two pilots, five flight attendants, and 40 passengers...
The Captain of Copa Airlines Flight 201 was 53 year old Rafael Carlos Chial, who had 23,750 flight hours with 7,000 of those hours being on the Boeing 737...
And the First Officer of Copa Airlines Flight 201, was 25 year old Cesareo Tejada, who had 3,450 flight hours with 1,600 of those hours being on the Boeing 737...
And as for the cause that wound up leading to the crash of Copa Airlines Flight 201?...
Well about 10 minutes after takeoff, Captain Chial contacted Panama City Air Traffic Control, and requested weather information. To which the controller reported that there was an area of very bad weather 30–50 miles from their position...
And at 8:48 pm, Captain Chial made another radio contact requesting permission from Panama City ATC to fly a different route due to the severe weather ahead. This new route would take the plane over Darién Province. And about minute later, at 8:49 pm, Panama City Control Center received a third message from Captain Chial, who wound reporting reaching cruising flight level of 250, or about 25,000 feet...
And the problems that would wind up downing Copa Airlines Flight 201 wound up starting shortly before completion of the return to course after diverting from the storm. When Captain Chial's ADI began to display false data, by only occasionally updating the bank angle, And this was due to a short circuit, which wound up confusing the pilots. And tragically, this wound up causing Flight 201 to bank the right and then entered a steep dive at an angle of 80 degrees to the right. And it also began to roll uncontrollably while accelerating towards the ground. And despite the attempts by both Captain Chial and First Officer Tejada to level off, the airplane continued its steep dive until it wound up exceeding the speed of sound, and started to break apart at around 10,000 feet...
And this then wound up leading to Copa Airlines Flight 201 to wind up crashing into a jungle within the Darien Gap at roughly 559 mph, with the impact killing all 47 onboard...
And to this day, Copa Airlines Flight 201 is the deadliest accident in Panamanian aviation history, and is the only fatal crash in the history of Copa Airlines...
And finally viewers, though I have said this in many of these same chapters before...
I will once again reiterate, that I don't mean any sort of disrespect, to those whose lives were lost by either this tragedy, or those like it...
